Attention Deficit Hyperactivity Disorder (ADHD)

Once you have had an appointment with your primary care provider, we ask that you fill out the following forms as directed and drop them off at the clinic for review:

For children: Parents, please complete the SNAP IV and WFIRS-P documents and have your child's teacher complete the SNAP IV and CADDRA-Teacher Assessment documents. There is a total of 4 documents that should be submitted to your primary care provider.
