For those patients affected by the pending practice closures on April 15th, your medical records will remain on our secured electronic network until requested otherwise.
We are required to safeguard patient charts in one capacity or another for a period of 16 years.

Each patient (or guardian) may request at any time to transfer their medical chart to a new provider (please allow at least 6 weeks for this). He/She/They may also request a personal copy of the chart in paper form or on unsecured USB. Please follow the steps below for this.
Chart transfer and/or copies are extremely labor intensive and this is not a service insured by MSP. However, our fees are meant to take into account those under financial hardship, as we are not here to make this already difficult situation even worse.
Please talk to our staff directly if you are concerned about the required fees.
Complete the following form if you want to transfer a chart to a new medical provider, or you are requesting a copy (paper or electronic USB) for yourself or household
Mail or drop-off form to: #120 -27 Helmcken Rd, Victoria BC, V8Z 5G5 or fax to: 778-265-3169.
Allow six weeks for processing. Once your chart(s) are ready for transfer or pick-up, you will be notified and prompted to pay the fee (cheque, credit card or debit only).
Once your chart(s) is ready for transfer to a new provider, you will be called to collect payment and we will then proceed with forwarding this onwards (staff will document in your chart that this was completed).
If you have requested an electronic USB copy or a paper copy of the chart(s), you will be notified once ready and must then come down to the clinic to:
-Process payment (cheque, credit or debit)
-Sign one of the following two waivers
-Physically collect the chart copy (paper or electronic)
