Drivers Medical Exam

General Information:

Driver's Medical Examination Report(s) are required by RoadSafetyBC in assisting them to determine if an individual is fit to drive. There are two versions of the Driver's Medical Form depending on the individual's age, medical conditions, and class of drivers license. Forms can be completed same day as your appointment, by our physicians at Eagle Creek Medical Clinic. 


Please be aware there is a 45 day deadline before a completed form needs to be submitted. When this examination is required you will receive a notice in the mail.


Once received please call us at (778) 265-1377 ext 5 to book your appointment with Dr. Ahmed or Dr. Elnagmy. At the time of booking we will ask you whether it is a blue form or yellow form.

What to expect during your appointment:

  • Review of medical history
  • Focused physical exam
  • Cognitive testing if necessary
  • Eye examination   

Please bring the following to your appointment:

  • BC Care Card 
  • Eye glasses (if needed for driving)
  • Any relevant medical information 
  • A list of current medications


Once completed our office will be able to fax your form to RoadSafetyBC for you.   

If there are any concerns once your Driver's Medical Exam has been submitted please contact us.
