Drivers Medical Exam

 General Information: 

The Drivers Medical forms are required by RoadSafetyBC in assisting them to determine if an individual is fit to drive. Currently RoadSafetyBC has 2 versions of the Drivers Medical Forms depending on the individual's age, medical conditions and class of drivers licence. 

When this examination is required you will receive a notice in the mail. Once received, please call us at (778) 265-1377 ext 5 and ask for Dr. Ahmed to book your appointment. At the time of booking we will ask you whether it is a blue form or yellow form.

These forms must be completed by Dr. Ahmed within 45 days of the individual receiving them.

What to expect during your visit: 

Please bring the following to your appointment:

• Your BC Health Card 
• Eye glasses (if needed for driving)
• Any relevant medical information 

During your appointment an eye exam will be done along with a focused physical exam by 

Dr. Ahmed. Once completed our office will be able to fax your form to RoadSafetyBC for you.
